At the hospital where Dr. Ethan works, a sickly 9 year-old patient, Emmy Rosario, develops a particular affinity and attachment to Dr. Ethan. The day has finally come where she receives her donor kidney. As it happens, the same night Emmy is due for her transplant is also the Miss International National USA pageant, where Ai Mei Shimmer is due to compete. Dr. Ethan has the night timed out perfectly. He avoids dinner with his dad, saying he has scheduled surgery. Ai Mei Shimmer will be able to walk the pageant with enough time to wipe away any trace of Shimmer and scrub up for little Emmy's kidney transplant surgery at 10pm.

At the pageant, bolstered by her fellow gaysians, Aunty Chan Kahmora Hall, and Eva Young, Shimmer easily impresses the crowd and is crowned Miss International National USA. The thrill is short-lived, as Dr. Ethan is being relentlessly paged. Emmy has gone into shock and only wants Dr. Ethan to tend to her. With no time to celebrate her victory, let alone transform out of drag, Shimmer rushes to the hospital, complete with doctor's jacket worn over her 20,000 Swarovski crystal gown, heels clacking. There is no mistaking that Dr. Ethan is in charge here, despite outwardly appearances. In the hospital hallway, Shimmer runs into Ethan's dad, bringing his hard-working doctor son a bag of Chinese food. Dad's face lights up seeing an Chinese face, speaking Chinese to Shimmer. Eva promises to give the food to Dr. Ethan. As the tall, striking Chinese woman walks down the hallway, barking orders to the nurse, dad notices a strange familiarity.
